COMMUNITY OPTIONS PROGRAM

    The Community Options program coordinates services in Manitowoc County to help elderly, ill or disabled people to remain or return to home.

What is the Community Options Program? (COP)
    COP offers alternatives and options so that persons with long term care needs are able to remain in their own home/community with necessary support services.

Who is Eligible?
    Manitowoc County elderly and disabled persons unable to remain at home without some type of assistance and who are at risk of entering a nursing home or are current residents of a nursing home or institution.

How is Eligibility Determined?
    An assessment is provided which gathers information about the person's current needs and capabilities.  Part of this assessment will involve a review of services and supports or other changes which would be helpful in making it possible to remain in the community.
    You, as the person being assessed, will be expected to provide needed medical, personal, and financial information.  There is no charge for the assessment.

After the Assessment, Then What?
    The key to COP is choice- your choice - regarding where you want to live and what services you want.  Once the assessment is completed, you make the decision on future services and/or living arrangements.
    If you decide to remain at home or return to the community, a plan will be developed to determine the kind of help you will need to live in the community.  There is no charge for the plan.
    The amount of help in paying for community services is dependent upon your income and assets.
